Hospice care is a type of health care that focuses on comfort, support, and medical assistance to terminally ill patients and their families.  In addition to the palliation of symptoms, hospice attends to the emotional and spiritual needs at end of life. Hospice care prioritizes comfort and quality of life by reducing pain and suffering. Hospice care provides an alternative to therapies focused on life-prolonging measures that may be arduous or are not aligned with a person’s goals of care. To qualify for hospice services a patient must be certified as terminally ill with six months or less life expectancy.

Our Services

Interdisciplinary Team

The hospice interdisciplinary care team is a group of healthcare professionals who provide wholistic care to patients with life-limiting illnesses and their families. The team includes access to a physician, nurse, hospice aide, social worker, volunteer, chaplain, and bereavement specialist.

Spiritual Care

Spirituality is a fundamental element to the human experience of health and healing, illness and dying. Spiritual care is an essential component of palliative and end-of-life care and supports patients and their families compassionately through this transition.

Medical Director

The Hospice Medical Director is a physician who provides clinical expertise and leadership to the Interdisciplinary Team to improve patient care and palliation during a patient’s time on hospice service.

Skilled Nursing

Skilled Nurses perform a full spectrum of nursing care, including catheter care, wound care, and medication management. Acting as a liaison with external providers, Skilled Nurses also teach patients and their caregivers how to manage patients’ medical conditions in the home environment.

Bereavement Support

Bereavement is the period after a loss during which you experience grief. Bereavement support is an essential part of the hospice philosophy, providing up to 13 months of support to help families and friends process grief after the death of a loved one.

Certified Nursing Assistant

Certified Nursing Assistants assist patients with activities of daily living such as bathing, dressing, grooming, oral care, hygiene, and toileting. They help to maintain dignity and comfort while respecting patient preference for care.

Social Work

Social Workers enhance the coping skills of families and caregivers while promoting the social and emotional health of the patient. Social Workers can facilitate understanding goals of care during a difficult time and can assist in services such as a living will or POLST.

Volunteers

Hospice volunteers are an essential part of the hospice team. Volunteers can provide direct companionship and bring unique talents, empathy, and assistance to patients and their families.

Pharmacy

Pharmacists play a critical role in hospice care by addressing complex symptoms, monitoring possible drug interactions, and avoiding overutilization of medications to ensure a quality end-of-life experience.
